First Visit
Thank-you for choosing our office for your periodontal care. At your first appointment we will
complete an in-depth medical and dental health history and exam to measure for bone loss, loose
teeth, bite
related problems, oral cancer screening and other signs of periodontal disease. We
reserve one hour for this appointment. A treatment plan and estimate of fees will be provided at the
end of your visit.

Please assist us at the time of your initial visit by providing the following information:
+ Your chief dental concerns
+ Referral form and x-rays from your referring dentist (if possible)
+ List of current medications
+ If you have dental insurance, please bring any insurance cards with you

Please notify the office if you have a medical condition such as artificial heart valve or joint or any
other medical condition that requires pre-medication; prior to treatment.

Our office is located at 8340 Mission Road Suite #203. We are located in the Corinth Office Building on the second floor.


Scheduling
We respect our patients time and will make every attempt to complete periodontal treatment in as
few visits as possible. We have found that since we are considerate of our patients time, they are
likewise considerate of our time. We require 48 hours notice to change appointments. If a Monday
appointment needs to be changed, we ask that our office be called by noon on Friday. This allows us
to care for someone else that needs our attention.

Emergencies
If a patient is in discomfort, this is our immediate concern. Please contact our office and we will do
our best to schedule you an appointment as soon as possible. Please note that medications can only
be prescribed to a current patient of record.

Insurance
The goal of our office is to provide the highest quality periodontal care at reasonable fees. These
fees will be discussed prior to any therapy.

We understand that finances are of concern. We are providers for the top dental insurance
companies and will gladly file with any of your insurance carriers. We are always ready to assist our
patients in obtaining the maximum benefits with their insurance plans. Our staff deals with hundreds
of insurance plans. Policies, plans and guidelines can change weekly. Some insurance companies will
not disclose fees until treatment is completed. Dealing with these companies can be difficult and time
consuming. It is important that you keep us informed of any changes to your insurance.

While we do our best to assist you with your insurance, please keep in mind that your insurance plan
is a contract between you(or your employer) and the insurance company. At no time is the insurance
company obligated to pay the periodontal practice even with pre-approval. Therefore, all fees are
ultimately the responsibility of the patient. We will file all claims on your behalf (electronically
whenever possible) the day of service. Any insurance disputes can be handled by contacting the
Kansas Insurance Commissioner at www.ksinsurance.org.

A variety of payment options are available including most major credit cards and interest-free
payment plans.
